Registration Procedure 

If you wish to register with our Practice, you should live within our practice area. The registration procedure and questionnaire can be obtained from reception or downloaded from here.

GMS1W Registration Form

Please follow the Registration procedures below:

ADULTS OVER 16

  • Complete The Corwen Family Practice Adult Registration Form and the GMS1W Registration Questionnaire.

The GMS1W Registration Form MUST been signed in the bottom left corner of the front page. We cannot accept it unsigned.

  • Both forms and ID can be dropped off at reception or emailed to enquiries.w91047@wales.nhs.uk
  • Your GMS1W form MUST be signed.
  • A copy of repeat prescription slip for your medications is required for our records.
  • Your NHS number: This is a 10-digit number, like 485 777 3456. You can find your NHS number on most documents or letters sent to you by the NHS such as prescriptions, hospital letters, appointments letters. If you are still not sure you can contact your previous practice for your unique NHS number.

Once the above documents and completed forms have been provided to the practice you may be offered a registration health check appointment (this may be completed via telephone).

CHILDREN UNDER 16

  • The GMS1W Registration Form to be completed by the parent/Guardian. Two forms of ID are requested.  The completed forms and ID can be dropped of at reception or emailed to enquiries.w91047@wales.nhs.uk
  • NHS number: This can be found in your child's Red Immunisation book. 

Unfortunately, there are circumstances which may result in a person being removed after registration and these are:

  • Any violent or aggressive behaviour, both verbal and physical towards staff.
  • Damage to or theft from the practice premises.
  • Smoking or the consumption of alcohol/illicit drugs on the premises or attending in an inebriated state.
  • Inappropriate use of the emergency out of hour’s service.
  • Recurrent non-attendance of appointments (including New Patient Health Check).
  • Recurrent inappropriate requests for house visits.
  • Any other behaviour that significantly affect the Doctor patient relationship.
